We are looking for the right person to join our Let’s Talk Team based in Jarrow Town Hall.
You will have experience of working in a customer related, person centred environment, having experience of handling enquiries and giving information and advice using multiple systems, working with adults with care and support needs, to deliver excellent outcomes for people.
You will be confident and experienced in supporting conversations with people face to face, to ensure they receive the right support to meet their needs, aimed at developing and maintaining their independence.
The Let’s Talk Team is the single point of contact into Adult Social Care in South Tyneside and the first point of communication for members of the public and professionals.
You will work as a team delivering effective customer focussed practice and be responsible for ensuring practice is safe, person centred and focusses on the strengths of people and their communities. You will ensure the support you provide is highly effective, provides excellent customer service, and meets all statutory requirements and working within a framework of effective performance management and quality assurance activities.
You will work alongside other team members to ensure people are supported in the most proportionate way to meet their needs, and to share good practice, and you will have close links to services in the local areas which are available to enable the Lets Talk Team to connect people to the right support, in the right place, at the right time.
