We are seeking a Telecare Coordinator with strong ethics and values to support adults and carers in our communities to live the life they choose to, regardless of their age or stage of life.

We have an exciting new opportunity for two Telecare Coordinator’s co-located with the Adult Access Team to co-ordinate the Telecare service offer from Adult social care, to ensure the most appropriate equipment is identified to meet assessed outcome and that this remains appropriate through timely reviews.

To support colleagues with fostering a tech enabled approach to strength-based practice, that will support people to live more independently in their home environment- in accordance with the principles of Prevent, Reduce and Delay onset of increased need as outlined in the Care Act 2014.
