An exciting opportunity has arisen to work within the experienced and supportive integrated community team based at Clarendon House. You will need to be committed to person-centred working and enjoy a fast-paced, multi-disciplinary environment.
You will provide high quality and timely social care support responses to the needs of service users and their carers, in line with statutory requirements within the Care Act and Council policies and procedures.
You will support the delivery of key areas of social work practice such as safeguarding, continuing health care, mental capacity assessments and best interest decisions within a multi-disciplinary setting.
