We are looking to recruit enthusiastic Senior Reablement Assistants at the intermediate care centre in eston.
We have a number of posts for both days and nights.
The primary role is to to participate in the provision of a Reablement Service in the Intermediate Care Centre to help prevent unnecessary hospital and care home admissions and to facilitate Early Supported Discharge from hospital.
The role is to assist with the provision of a Reablement service to enable individuals to regain or maintain their ability to live independently as far as possible.
To participate in the monitoring and management of complex health conditions through the measurement and supervised treatment of baseline vital signs to enable people to be cared for at home.
The ideal candidates will be educated to Level 3 standard and have experience with admisnitering medications.
Night role is subject to enhanced rates of pay
