An exciting opportunity has arisen to join the Community Reablement Team for the reablement and support of people within the  Redcar and Cleveland Area.

Join us in providing essential, rapid response care within the comfort of a person's home for up to seven days. The rapid response service makes a real impact in people’s lives preventing hospital and care home admissions alongside supporting hospital discharges. The team also supports people in their own homes for up to 6 weeks helping them to regain their ability and confidence to live independently.

ABOUT THE JOB
  • Various weekly working hours and rotas’ s are available.
  • Opportunities for Career Progressions within the service are available. 
The successful candidate will have the ability to meet the travel requirements of the post. Working in people's homes within the Redcar and Cleveland community areas and being able to commute to work and home.
