Through the Better Care Fund we have an ambition to provide better integrated health and social care, care and support services for the adult population of Stockton.

This post will work across health and social care in Stockton. Working with Hartlepool and Stockton on Tees CCG and the Council to develop the Better Care Fund plan and then to work with providers, including the Voluntary Community and Social Enterprise (VCSE) sector, to develop services.

You will have knowledge and experience of working in a Health or Social care setting. The role will involve the development and monitoring of a programme to implement the Better Care Fund priorities and to oversee the performance management of the BCF. The post will also be involved in the design of pathways and processes within health and social care services to develop an integrated service delivery approach which centres on “the customer journey”.

Experience of performance management frameworks is essential as is good analytical and creative skills together with the ability to present information in various formats. The role will include identifying and developing ICT solutions and as such a knowledge and understanding of ICT and Information Governance will be needed.

This is a transformation role change services for people requiring health and social care interventions, it will be both challenging and rewarding as we strive for better care services for those in need.
