We have a full time Social Worker vacancy within Easington Locality Team.
The Team enjoys excellent working relationships with colleagues in Health and other statutory and non-statutory organisations.
The Social Worker is responsible for completing assessments of need, care planning and reviewing and working within multi-disciplinary settings for Adults with a physical disability and Older People.
It is expected that the successful candidate will have an up to date knowledge of multi-agency working and the associated adult care agenda.
It is anticipated that interviews will be held on 19th December 2016.
Required Qualifications: DipSW or recognised equivalent, current HCPC Registration, together with experience of working with the client groups serviced by the team.
