We are looking for a reliable self-motivated person to provide support to the Local Authorities of Stockton and Hartlepool along with Hartlepool and Stockton Clinical Commissioning Group in the delivery of the Learning Disability and Autism Transforming Care agenda. To inform improvements in the planning of care and support, that will advise the commissioning requirements for Health and Social Care, with the aim of facilitating a reduction in the reactive commissioning and crisis management.

The key aim is to establish and maintain a detailed understanding of the children, young people and adults who have been identified as being admitted to, or at risk of being admitted to, inpatient services. There will be a focus on the requirements of young people and transitions now and in the future as well as adults.

Previous experience of working in a social care setting would be an advantage.

The post holder will be required to communicate across all levels within the organisations including external partners and agencies.
