We have an exciting opportunity for a motivated, hardworking and flexible individual to join our team as Health and Social Care Coordinator

We provide a specialist respite and crisis service that supports adults with learning disabilities with additional complex support needs within a building base. The service enables families to have a short break while we support their family member within the service.

The role involves supporting, supervising and co-ordinating members of the staff team .
Maintaining the dignity of people must always be a priority and successful applicants must be able to deliver care and support with sensitivity.

You will need to be able to undertake assessments of the needs of the people accessing the service in order to develop, implement and review care plans.

You will need to evidence that your practice ensures that the needs and aspirations of the individuals we support are met and support people to express themselves, make choices and utilise their own unique forms of communication/expression

In addition, you will receive:
